Adding Respiratory Therapy in-Facility Increases Positive COPD Patient Outcomes

  • Given that patients with COPD have some of the highest comorbidities (e.g., congestive heart failure, acute renal disease, pneumonia), identification of patients with COPD is most challenging.

  • With the addition of COPD to the Hospital Readmissions Reduction Program, the Patient Protection and Affordable Care Act also changed the formula for readmission to include all causes and all conditions.      *Several previous investigations have evaluated the cost-effectiveness of respiratory care protocols and devices. Each of these, involving both COPD patients and other diagnoses, revealed opportunities for reducing operational costs and demonstrating the value of RTs.   They created a clinical scoring tool that was incorporated within a patient assessment and protocol program, with the goal of improving the quality of care for COPD patients and assessing the ability to decrease operational costs.

  • Twenty percent of Medicare-eligible patients (over 65 y old) admitted for COPD exacerbations were reported to require readmission within 30 d.1 Half of these readmissions were for respiratory-related disease.

  • The study by Silver et al. 5 is especially relevant to the essential role of respiratory therapists in preventing readmissions following COPD exacerbation. These investigators randomized 428 subjects at high risk for readmission to usual pre-discharge care versus a comprehensive respiratory therapy disease management program conducted by specially trained respiratory therapist case managers.

  • The respiratory therapy disease management protocol incorporated pre-discharge subject education.

  • Key findings of the study were that subjects receiving respiratory therapy disease management experienced significantly fewer readmissions for COPD exacerbations (20% vs 28%), shorter duration of hospitalization if readmission was required (5 d vs 8 d), and fewer total inpatient and ICU days over 6 months of follow-up.
